Purpose of this form
This form obtains your consent to participate in telehealth appointments with Natural Terrain Therapy. Please read each clause carefully before signing below.
Records
Telecommunications with clients will not be recorded or stored. Medical information obtained during diagnosis and analysis may be used anonymously for further improvements in scientific studies.
Information
Medical information related to your history, records and tests will be discussed during the telehealth appointment with video and audio.
Access
You will need access to a PC, laptop, or mobile device and a reliable internet connection in order to have an efficient telehealth appointment.
Your rights
You may withdraw your consent at any time without affecting future sessions. You are welcome to ask questions related to telemedicine appointments and technical requirements at any point.
Privacy
All laws protecting your privacy of health history and information apply equally to telehealth practices. Your information is handled with the same care and confidentiality as in-person appointments.
